Kindergarten registration is held at
PRIEST STREET SCHOOL
115 Priest Street
Leominster, MA 01453
on TUESDAY, WEDNESDAY AND THURSDAY’S FROM 9:30 -1:30
UNTIL June 30, 2008.
PROCEDURE
INSTRUCTIONS
Parent or guardian should bring the following at the time of
registration:
1. A birth certificate or copy from a municipal governing
agency.
A hospital birth record/baptismal certificate is not
acceptable.
2. An immunization record or copy from a medical doctor with
all mandatory immunizations recorded. The School
Immunization Law (C.76.S. 15. M.G.L.) requires that children
have 5 doses of DPT (Diptheria, Pertussis & Tetanus), 4
doses of Polio, 2 doses of MMR (Measles, Mumps & Rubella), 3
doses of Hepatitis B and 1 dose of Varicella or Physician
verified history of Chicken Pox before starting
Kindergarten.
3. Evidence (the date and results of the lead test) that the
child has been previously screened for lead poisoning.
(Massachusetts Department of Health Regulations 105 CMR
460.000 requires that children present evidence of having
been previously screened for lead poisoning as a condition
for entry to kindergarten.)
4. A copy of a Physical Exam dated after September 2007 or
proof of upcoming appointment.
5. Proof of Residence (i.e., top part of utility bill).
It is not necessary for parents or guardians to bring their
children with them at the time of the registration.

Padres o guardianes deben de traer la información siguiente
al tiempo de la matriculación:
1. Un Certificado de Nacimiento
Record de nacimiento del hospital/Certificado del Bautismo
no es aceptable.
2. Un registro de inmunizaciones o una copia del medico con
todos las inmunizaciones requeridas. La ley de
inmunizaciones de escuela (C.76.S. 15. M.G.L.) requiere que
los niños tenga 5 dosis de DPT (Difteria, Tos Ferina, y
Tétano), 4 dosis de Polio, 2 dosis de MMR (Sarampión,
Paperas, y Rubéola) 3 dosis de Hepatitis B y 1 dosis de
Varicela o verificación del medico de historia de varicela
antes de empezar el Kindergarten.
3. Evidencia (la fecha y resultado del examen de plomo) que
el estudiante ha sido investigado para el envenenamiento de
plomo. (Regulaciones del Departamento de Salud de
Massachusetts 105 CMR 460.000 requiere que los estudiantes
presenten evidencias de haber sido investigado previamente
para el envenenamiento de plomo como condición para entrar
al Kindergarten.)
4. Una copia del Examen Físico obtenido después de
septiembre del 2007 o prueba de una cita para el Examen
Físico.
5. Prueba de Residencia en Leominster (por ejemplo; una
factura de energía o del gas)
No es necesario para padres o guardianes a traer el
estudiante con ellos al tiempo de la matriculación.

Integrated Special Needs Preschool
Program
Program Description
The Leominster Public School integrated preschool program is
designed to provide services to 3 and 4 year old special needs
students in classes with students with no special needs.
The program provides the children activities in the areas of
cognitive development, fine and gross motor skills, language
development and social skills.
Purpose of Program
The purpose of the program is to provide an early childhood
program for children with a variety of learning and social
skills. The program provides a natural setting for children to
learn to play together and learn through a series of
developmentally appropriate activities.
Facts about the Program
1. Integrated children attend the program for 2, 3, 4 or 5 half
days per week. There are a few 5-day full day slots and 1 class
of 4 full days and ½ day on Friday.
2. Parents are responsible for transportation, to and from the
program.
3. The Programs are at Bennett and Samoset School morning and
afternoon sessions.
4. Integrated children must have no special needs and
participate in a screening.
5. Integrated children must be Leominster residents.
6. Once accepted into the program, children must be registered
through the Leominster Public Schools.
7. Children are chosen by a lottery system, children must be 3
or 4 years old by August 31st. If slots are still available
after the lottery they are given out on a first come first
served basis. Applications for September 2008 are available in
the Special Education Office starting April 1 from 8:30AM to
4:00PM.
8. Tuition is non-refundable, if your child is sick and does not
attend or if you
move out of Leominster or withdraw from the program, there are
no refunds.

PAYMENTS MUST BE MADE BY CHECK OR MONEY ORDER
2-day week, ½ day session, $180 due in 4 payments,
June 1, November 1, February 1, and April 1, 2009. $
720.
3-day week, ½ day session, 6 payments of $180: June
1, October 1, December 1, February 1, March 1, May
1, 2009. $1080.
4-day week, ½ day session, 8 payments of $180: June
1, October 1, November 1 ,December 1, February 1
,March 1, April 1, May 1, 2009. $1,440.
5-day week, ½ day session, 9 payment of $180: same
schedule as 5 day full, $1,620.
4 day full M-TH ½ day Friday, 9 payments of $293,
total $2,637, same schedule 5 day full.
5 day week, full day session, $320 a month due on/or
before the first of every month (9 Payments) June 1,
,October 1,November 1, December 1, January 1,
February 1, March 1, April 1 and May 1, 2009.
$2,880.
Payments are the responsibility of the parents. If
your child is not up to date on payments they will
not be allowed to attend.
Tuition payments over one week late will be charged
$5 late fee for each week that payments are late. |
